SMS Terms & Opt-In Policy
How Golden Gate Health collects consent for text messages, what we send, and how to opt out at any time.
Golden Gate Health ("we," "us," or "our") offers an SMS text messaging program to keep patients informed about their care. This page describes the program, how we obtain consent, the types of messages we send, and your rights as a participant.
Program Description
Patients who opt in will receive text messages from Golden Gate Health related to their care, including:
- Specialist referral updates and instructions (e.g., referral status, specialist contact information, and scheduling details)
- Appointment reminders and confirmations
- Requests to call our office regarding your care
- Follow-up and care coordination messages
Message frequency varies based on your care needs; most patients receive a few messages per month. Message and data rates may apply depending on your mobile carrier plan.
How We Collect Consent (Opt-In)
Golden Gate Health collects consent for text messaging verbally, directly from the patient (or the patient's parent or legal guardian).
- At your first appointment — our front office staff will ask you whether you would like to receive text messages from Golden Gate Health. Staff may also collect your consent over the phone.
- If you agree — we will send text messages to the phone number you provide from our dedicated texting line, +1 (877) 522-0590.
- Cancel at any time — reply STOP to any text message, or simply tell our front office staff the next time you come in, and we will remove you from the program.
Consent is not obtained through online forms, purchased lists, or third parties, and enrollment in text messaging is never a condition of receiving care.
Verbal Opt-In Disclosure — Read by Staff
"Golden Gate Health would like to send you text messages about your care, including referral updates and appointment reminders. Message frequency varies, and message and data rates may apply. You can opt out at any time by replying STOP or by letting our front office staff know, and you can reply HELP for help. Consent is not required to receive care from us. Do you agree to receive text messages from Golden Gate Health at the phone number you provided?"
We only send messages to patients who have answered "yes" to this disclosure. A record of the patient's consent, including the date it was given, is maintained in the patient's file.
How to Opt Out
You can cancel the SMS service at any time by texting STOP to +1 (877) 522-0590 or replying STOP to any message you receive from us. After you send STOP, we will send you a final message confirming that you have been unsubscribed, and you will no longer receive SMS messages from us. You can also opt out by telling our front office staff at any visit, and we will remove you from the program. To rejoin, simply let our staff know at your next visit or call us at (909) 823-8000, and we will re-enroll you.

Privacy
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. We share your information only with service providers that deliver messages on our behalf (such as our SMS platform provider), as required to operate the program, or as required by law. Carriers
Carriers are not liable for delayed or undelivered messages. Supported carriers include all major U.S. carriers.

We may update these SMS terms from time to time. The most current version will always be posted on this page.
